    Sorry for the inconvenience.

    By the way, this issue raised for double loading the Bootstrap script.

    Please uncheck the Bootstrap Load check box from Pop Forms Setting -> Scripts Load to fix the issue.

    Please follow the steps from the documentation that we have provided with the main_files.
