The plugin is a premium plugin https://codecanyon.net/item/virtual-phone-number-selling-form-wordpress-plugin/19936185 developed by our team that we are providing free with this theme. However, based on the plugin development process the fields can not be deleted because there have connections with other options in the plugin used on the front-end to select a number and options. If you want to delete the fields it will look like you need to re-build the plugin based on your needs.

It’s not our theme issue, it’s all about the plugin issue as you can see the Warning indicated there was an issue in line number 738 of the hooks.inc.php file of the plugin.
